Health plans which we accept:
We participate in most insurance plans, including Medicare. If you are not insured by a plan we do business with, payment in full is expected at each visit. If you are insured by a plan we do business with, but don’t have an up-to-date insurance card, payment in full for each visit is required until we can verify your coverage. Knowing your insurance benefits is your responsibility. Please contact your insurance company with any questions you may have regarding your coverage.

Blue Cross Blue Shield (Not Blue Select or My Blue HMO)
Federal Blue Cross Blue Shield
United Healthcare AARP (out of network benefits)
Cigna and Cigna Baycare Plans
All other plans we accept out of network benefits and are in process of getting in network. If you do not see your plan listed here please call us or your insurance company to find out if we are in network.
We have very reasonable cash pay rates depending on services required.

Please arrive at least 15 minutes prior to your scheduled appointment time to complete the registration process.
To facilitate your registration, please bring the following information with you:
Your insurance card and identification
List of your current medications
Names of other physicians you have seen
Completed new patient forms
If you are unable to make your appointment, please notify us 24 hours in advance.
